I recently published a March 10, 2025 blog post about the form. You can find that article here. The form is essentially divided up into two sections: belief statements and accoutrement. The belief statement section is further divided into two sections with one discussing overall or general beliefs and then discussing sacrament-specific beliefs and practices.

The accoutrement section has ten subsections which mirror the Wyoming District Court’s opinion in Meyers, the test upon which the form is fashioned. Again, there is more detail about the form and its origins.

Included inside the form is an example language covering each section of the Meyers test. I placed this language in the form as a way of illustrating both nature and the form of the information that should go into each individual section. IF a client’s beliefs and/or practices match or closely resemble those expressed in the example language, they are free to use my form language as well.

Please note, this form has been implemented to lesser or greater degrees in over 100 entheogen-based religious organizations in the United States. As such, to the extent my form, and even my form language is used, your church will be in accord with an already established network of churches with the same or substantially similar beliefs. Ultimately, the more churches

subscribing to a certain set of beliefs, the easier it will get for courts to rubberstamp the religion question in favor of our fellow community members and children. We provide yearly church maintenance packages and can review your Statement of Beliefs yearly with your church board to discuss what, if any, edits or revisions might best be made.
